A program participant survey asks the people your services exist for how they were treated and whether the help worked for them. This template takes about a minute in plain language: a face for the visit, four statements on respect, usefulness, access and knowing what to expect, and one open question on what to change.
It is written for the end of a visit or a service, shared as a QR code on a sign at the front desk or on the appointment card, so people answer on their own phone once they have left. The same link works in a follow-up email your own system sends, and anonymous responses hold for link or QR code surveys like this one.
Every written answer is read as it arrives, in any language, and filed under a category from your list, such as programs and services or communication, with a type, a sentiment and an urgency. Results shows the share of each face and a heat map of the four statements with row averages, against the period before.
